Dentist

REPORTS TO: Dental Director

JOB SUMMARY:
Responsible for delivery of quality dental care to patients of the Center. Performs professional dental care in the diagnosis and treatment of patients; assists Dental Director in developing and implementing the dental program.

QUALIFICATIONS:
Graduation from an approved School of Dentistry License from State of PA BLS certified

SPECIFIC DUTIES AND RESPONSIBILITIES:

1. Examines individuals requesting care, diagnoses their dental/oral conditions, prescribes and carries out, or directs others in carrying out appropriate dental/oral treatment, or refers individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines.
2.  Records patient-dentist transactions as they occur in the patient’s dental record to accurately and completely reflect the nature of the contact, the condition of the patient, and the care or treatment provided.
3.  Cooperating with professionals within both the Dental Department and other departments of the Health Center, and within the community-at-large to meet the needs of patients of the Health Center.
4.  Providing education and training for Health Center staff in areas of particular expertise with respect to the practice of dentistry.
5.  Participating with other appropriate personnel of the Health Center in the implementation and operation of an electronic dental record.
6.  Participating in conjunction with other appropriate personnel of the Health Center in the establishment and implementation of a sufficient dental data management and retrieval system.
7.  Serving as a representative of the Health Center before the dental community, professional organizations, public regulatory and administrative bodies, and within the community-at-large, as requested.
8.  Confer with and advise the Health Center’s CEO and Dental Director concerning any rule, regulation or policy involving the Center’s Dental Department.
9.  Performing in a timely and satisfactory manner all acts consistent with the Dentist’s duties and responsibilities as indicated in the Health Center’s Employee Policy Statement and the Dentist’s job description as in 3 above.
10.  Undertaking all actions necessary or proper for the carrying out of any of the above described duties and responsibilities. In performing his/her functions in fulfillment of the aforementioned duties and responsibilities, the Dentist shall observe and comply with such rules, regulations and policies as may from time to time be determined by the Health Center’s Board of Directors, CEO or Dental Director and communicated to all the Dentists in writing.
11.  Actively participate and contribute to the QA/QI Committee.

Licensed Clinical Social Worker (LCSW)

REPORTS TO: Chief Medical Officer

JOB SUMMARY:
This position is responsible for the diagnosis and treatment of mental and emotional disorders, including disorders of mood and addiction, aimed at the prevention and management of such disorders; provides consultation and psychotherapy for individuals, couples, and families. The philosophy of recovery/wellness will be utilized by this clinician.

QUALIFICATIONS:
Master’s Degree in Social Work required. Pennsylvania Social Work licensure required. Knowledge of the basic concepts and principles of managed care required. Knowledge of community resources required.

SPECIFIC DUTIES AND RESPONSIBILITIES:

1. Works in collaboration with administration and the Medical Director in the design, implementation, and evaluation of the program objectives, including the development of quality initiatives.
2.  Lead in identifying appropriate tools, consent processes, documentation and development of new policies/procedures for the behavioral health service line to ensure the highest level of compliance standards are met.
3.  Work to maximize use of NextGen for behavior health service line and completes required documentation in a timely manner.
4.  Counsel individuals, group, or families to help with mindfulness, manage crisis situations, define treatment goals, and develop realistic treatment plans with a focus on recovery.
5.  Collect information about individuals or clients, using interview, case histories, observational techniques and other assessment tools and Provider referrals across all programs and reference evidence based resources to identify symptoms and manage treatment plans.
6.  Coordinate care with Clinical Providers to insure continuity of care and determine barriers to care that are obstacles to compliance by attending weekly high-risk huddles.
7.  Coordinates referrals between and among physical, behavioral, healthcare providers and other community resources to improve overall client outcomes. Ensures appropriate clinical management information is shared with peers, providers and outside agencies in a timely fashion while securing system privacy standards.
8.  Make referrals to a medication assisted treatment program if indicated
9.  Other duties as assigned

JOB SUMMARY:
Provide assistance to Physician and Nurse as requested. Compile and document patient medical history, administer necessary screenings, collection and preparation of specimens for transportation to lab, assist with treatments, procedures, administration of medications, patient education, phone communication, document all patient information in patients’ chart.

QUALIFICATIONS:

SPECIFIC DUTIES AND RESPONSIBILITIES:

1. Assist physician with exams, treatments, procedures and collections of specimens and samples
2.  Obtain and record the patient’s vitals along with chief complaint and brief interim health history and prepare patient for examination.
3.  Perform lab tests, vision screening, audiometry, throat cultures, glucometer readings, HbA1C, lead screenings, EKG and other testing as required
4. Administer and record medication and immunization as ordered by the physician
5. Maintain on-going communication with patients (and family) via telephone as necessary
6. Triage all incoming patient telephone calls
7. Coordinate and delegate timely processing of referrals as directed by the physician
8. Clean exam room after each visit, prepare and stock exam room next patient visit
9. Order supplies and medications for suite, monthly or as needed
10. Assist Physician and Nurse in completing necessary forms such as, but not limited to medical record requests, insurance forms, physical forms, driver license forms, etc.
11. Dispose of contaminated and disposable items
12. Identify Performance Improvement opportunities
13. Participate and collaborate with the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a plan of care
14. Administer injections as directed
15. Perform data entry as requested in various registries, but not limited to diabetes, cardiovascular, births, SIIS, etc.

1. Cooperate in conjunction with the CEO, Medical Director and officers of the Board of Directors of the Health Center, in establishing and implementing procedures to insure that high quality medical care is provided by the Center, and mechanisms to properly evaluate the quality of medical services being rendered by Center personnel.
2. Perform direct patient care to include examination, diagnostic testing, treatment and post-treatment monitoring of patient progress as needed.
3. Cooperating with professionals and paraprofessionals within both the Medical Department and other departments of the Health Center, and within the community-at-large needed to meet the needs of patients of the Health Center.
4. Providing education and training for others associated with the Health Center in areas of particular expertise with respect to the practice of medicine.
5. Participating with other appropriate personnel of the Health Center in the implementation and operation of a Medical Records Department.
6. Participating in conjunction with other appropriate personnel of the Health Center in the establishment and implementation of a sufficient medical data management and retrieval system.
7. Serving as representative of the Health Center before the medical community, professional organizations, public regulatory and administrative bodies, and within the community-at-large.
